A Governed Action
A Governed Action is one external-agent high-responsibility write that First Instinct receives, freezes, evaluates against a versioned policy, and records so it can be retrieved later.
It is billed on every outcome — allow, deny, require-human-approval and indeterminate — because all four consume the same evaluation and record infrastructure, and indeterminate is often the most operationally valuable. Retries, polling and replaying the same record are not billed again.

Sandbox actions are R0. Receiver and Target show as not_connected and cross-party assurance is incomplete — the value appears once a counterparty connects.
Third-party costs
Mobile-operator, KYC, payment and other third-party query costs are passed through at cost, or pre-committed. First Instinct does not mark them up or run on reselling them.
